Job Description: - Educates members on their care team and assists with coordination of care across the continuum, including scheduling appointments and referrals.
- Participates in daily operations of the practice, including answering phone calls, assisting with front desk inquiries, and maintaining the clinical space. Communicates through various digital channels to support patient care.
- Performs initial health screens and intake for new members. Obtains and reviews old medical records to ensure comprehensive patient history.
- Identifies care gaps and orders necessary screening tests. Confirms appointments and provides instructions to patients.
- Conducts pre-visit RAF reviews to prepare for patient visits. Welcomes patients, provides tours of the practice, and reviews welcome packets. Ensures all medical records are available for office visits.
The expertise and experiences you'll need to succeed: QUALIFICATION REQUIREMENTS: Technical/Vocational School (Required)Basic Life Support - CPR Cert (BLS) - RQI Resuscitation Quality Improvement, Certified Clinical Medical Assistant (CCMA) - EV Accredited Issuing Body, Certified Medical Assistant (CMA) - EV Accredited Issuing Body, Clinical Medical Assistant Certification (CMAC) - EV Accredited Issuing Body, National Certified Medical Assistant (NCMA) - EV Accredited Issuing Body, Nationally Registered Certified Medical Assistant (NRCMA) - EV Accredited Issuing Body, Registered Medical Assistant (RMA) - EV Accredited Issuing Body
Pay Range: $17.11 - $27.38
