Facing Forward to End Homelessness (Facing Forward) ends the cycle of homelessness for families and individuals by offering hope, help, and housing. We first ensure that our clients have a safe, stable home, and then we empower them as they lead a forward-facing life. We also offer case management, healthcare, referrals for substance abuse treatment and mental health issues, education, and job training. Facing Forward gives the tools our clients need to reclaim their lives.

COMPENSATION AND HOW TO APPLY

  • Salary range is $47,323.73 - $52,581.92 depending in experience and skills.
  • Benefits include medical, dental, vision, life, and long-term disability insurance. Employer-matched retirement plan and generous paid time off

SCHEDULE

  • This position is a hybrid position, alternating remote work, field work, and office days. Free street parking.
  • 9 am-5 pm, Monday-Friday, evening and weekend work as needed.

SUMMARY OF RESPONSIBILITIES

The Case Manager is responsible for

  • Provides comprehensive home-based case management services to all residents and their families to support stabilization in permanent housing, maintenance or increase of income, and increase in skills and resources.
  • Complete intake, assessments, service plans, annual progress assessment, benefits assessment
  • Provide life skills instruction in formal and informal settings, including, but not limited to financial literacy, tenant education, parenting education, and health/wellness education, and other skill development as needed.
  • Collaboratively develops and updates individual service plans with clients, provides encouragement, education, and advocacy toward clients’ individual goals.
  • Documents all client contacts and all services provided in progress notes.
  • Maintains updated paper files in neat and legible condition.
  • Completes all required paperwork in an accurate and timely manner, including but not limited to assessments, service plans, proof of income, releases of information, documentation of referrals, and updated proof of income and benefits for all household members.
  • Completes timely and accurate data entry into HMIS and any additional database as needed according to Facing Forward Data Entry Policy.

Sitting for extended periods is required. Intermittent physical activity, including standing, walking, and bending.
