Job Description & Requirements Schedule: Monday–Friday | 8-hour shifts | 40 hrs/week

What You’ll Do:
Manage 15–20 patient cases daily
Coordinate complex discharges across medical units
Collaborate with a strong social work team

Work alongside 30 supportive CM/SW professionals
Requirements:
✔ Active RN license (will wait for MS)
✔ 2 years acute care case management experience
✔ EPIC experience
✔ BLS or CCM
