Shift: Days, 8a-5p, 8x5, 40hrs per weekLength: 26 weeks Requirements: 

• This state is part of the PT Compact privilege states
• 1 Year of experience
• Outpatient rehab experience
• BLS
• Bachelor’s Degree in Physical Therapy is required. Master’s or Doctorate Degree in Physical Therapy is preferred
• Previous work experience providing direct patient care in outpatient, inpatient or inpatient rehab unit preferred
• Prior use of electronic medical records (EMR), preferred

Floating may be required (60) miles of the original assignment location 
Details:COVID-19 Initial Shot(s) • All workers, including remote, must be fully vaccinated. • Fully vaccinated is defined as 2 weeks after the completion date of a o 1 or 2 dose monovalent vaccine series, or o 1 dose bivalent vaccine • Exemption for Medical or Religious are reviewed by the client and must be completed using the Centura COVID Exemption form located in Egnyte. • Pfizer, Moderna, J&J, AstraZeneca and all World Health Organization accepted vaccine manufacturers are acceptable.
