Job Details
- The Pharmacist works in collaboration with the Director of Pharmacy and compounds and dispenses medications and pharmaceutical supplies upon receiving written prescriptions from physicians.
- Participates on Performance Improvement plans within the hospital.
- Consults with and advises Administration, Medical Staff and hospital staff on quality, regulatory and risk issues within the organization.
- Supports and serves as consultant to all clinical and Medical Staff.
- Assists the Director in coordinating and implementing the activities of the Pharmacy.
- Interprets medication orders, compounds and dispenses medication and other pharmaceutical preparations in accordance with legal, ethical and professional standards of pharmacy practice.
- Supervises the technical support staff.
- Assists with clinical, quality management and education programs.
- Provides support for all departments with respect to the care of patients aged 18 through geriatric population (65 and older).
- Participates in maintenance and dissemination of drug information through in-service education of hospital personnel, consultation with patients, hospital and Medical staff.
- Directs activities of and provides professional and technical guidance to Pharmacy personnel.
- Exercises professional judgment in imparting therapeutic and drug information to patients and health professionals.
- Capable of entering a minimum of 40 orders per four hour shift.
- Generates required reports including Pick I.V. and Production reports.
- Performs Backup Procedures correctly.
- Assists Technicians in performing technical tasks and maintains aseptic techniques.
- Recycles product as appropriate and checks technician prepared products prior to release.
- Ensures Technician maintains I.V. refrigerator temperature and hood cleaning logs.
- Fills orders with correct medication and checks support staff accuracy prior to medication release.
- Prioritizes work to prepare STAT, Now and Waiting orders in order of importance.
- Reviews patient profile to ensure appropriate and safe drug therapy, preventing therapeutic duplication, significant drug interaction, and avoidable adverse drug reactions.
- Medication ordered is appropriate for patient height, weight, age, renal function and disease state.
- Has knowledge of patient population served including pharmacokinetics dosing service, interpreting levels, modifying doses, and writing progress notes per protocol.
- Capable of calculating fluid, electrolyte and calorie needs and monitors patient appropriateness for formulation.
- Clarifies controversial orders with physician and keeps current on new drugs.
- Adds additives in proper sequence, performs syringe and vial counts, labels containers accurately including auxiliary labels, completes packages including seals and protective wraps.
- Maintains security of Pharmacy and controlled substances.
- Informs staff of pending problems and physical call back.
- Prepares unit of use dosage when possible.
- Keeps Director informed of workload status.
- Candidate will float between Kindred Aurora and Kindred Denver facilities approximately 3 days Aurora and 1 day Denver, subject to change.
Job Requirements
- Must have Colorado Board of Pharmacy (BOP) license attached to submission.
- Must have BLS certification through the American Heart Association attached to submission.
- Must have 1 year recent Pharmacy experience in a hospital setting preferred.
Schedule Information
- Shifts are 0800-1800 Monday through Friday.
- Shifts are 0900-1500 Saturday and Sunday.
- Call required.
