Your Role and Impact (The Core of the Job)

 

Your focus will be on performing high-value preventive care for the geriatric population.

Patient Care & Load

• Complete Medicare Annual Health Assessments (AHAs) through a mix of clinic and home visits.
• Manage a focused daily patient load designed for quality over quantity:
• • 7–8 patients/day in the clinic

Key Clinical Responsibilities

• Perform detailed Histories & Physicals (H&Ps).
• Utilize cutting-edge preventive screening tools (e.g., PAD and Neuropathy tools, diabetic retinal cameras, A1c and kidney testing).
• Master EMR systems and electronic documentation with provided training.
• Crucially: You will enhance patient outcomes through detailed evaluations and effective communication, collaborating with patients' primary care providers to ensure seamless continuity of care.

Requirements

• Current Nurse Practitioner (NP) license for the state.
• Comfort with using computers and electronic documentation systems (we'll teach you the EMR).
