MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

Schedule: Monday–Friday | 7:00AM–3:00PM or 8:00AM–4:00PM
Bilingual Preferred: Haitian Creole / English

\nSevita is excited to announce an opportunity for a Licensed Practical Nurse (LPN) to join our Adult Day Health team in Dorchester! We are seeking a reliable, compassionate nursing professional with a strong clinical background and a commitment to providing high-quality care.

\n \n Key Responsibilities \nMonitor the health and well-being of program participants, ensuring the highest quality of care.
\nAdminister medications, maintain accurate documentation, and oversee staff who assist with medication administration.
\nProvide prescribed medical treatments and personal care in accordance with physician’s orders.
\nConduct assessments, develop and implement health care plans, and respond appropriately to changes in condition.
\nTrain staff on topics such as infection control, universal precautions, and medication administration.
\nServe as a liaison and advocate between participants, families, physicians, and community service providers.
\nEducate participants on topics such as self-medication, smoking cessation, and STD prevention.
\nCurrent Massachusetts LPN or LVN licensure required.
\nStrong assessment, communication, and clinical documentation skills.
\nAbility to work collaboratively with staff, participants, and families.
\nPrior experience in an adult day health, community health, or long-term care setting preferred.
