This full-time Hospice and Palliative Care Physician role based in Hartford, Connecticut involves developing individualized care plans, coordinating with interdisciplinary teams, and overseeing hospice patient care in both inpatient and home settings. Palliative Care Physician opening Äì Hartford, Connecticut Position Details

  • Full-time Hospice and Palliative Care Physician role
  • Practice in a progressive, physician-led environment with nationally respected colleagues
  • Network includes seven acute care hospitals, large academic and surgical hospitals, thriving community teaching hospitals, and a multi-specialty group of 1,000+ providers
  • Schedule: 48 on-call weeks per year, Monday 7 a.m. ÄìFriday 7 p.m., including mandatory in-person attendance at 1 Interdisciplinary Team (IDT) meeting per week
  • Four weekends per year: Friday 7 p.m. ÄìMonday 7 a.m.
  • Key Responsibilities
  • Participate in admission decisions for hospice at home or inpatient settings
  • Develop individualized care plans as a member of the Interdisciplinary Team
  • Certify and re-certify patients eligible for hospice care under the Medicare Hospice Benefit
  • Consult with attending physicians regarding pain and symptom management
  • Coordinate care with attending physicians to ensure continuity
  • Develop and monitor the medical component of the hospice program
  • Participate in hospice service and policy development
  • Support staff and volunteer training initiatives
  • Conduct home visits (in person and virtual) as appropriate
  • Serve as a liaison and advocate for hospice with national, state, and local physicians, healthcare professionals, partners, and the community
  • Perform related administrative functions as required
  • Qualifications
  • MD or DO, Board-Certified/Board-Eligible in Hospice and Palliative Care or relevant specialty
  • Eligible for licensure in Connecticut
  • Strong communication, collaboration, and patient-centered care skills
